This agreement releases the Dravet Syndrome Foundation from all liability relating to injuries that may occur at the Super Siblings Camp, held during the 2023 Day of Dravet workshops. By signing this agreement, I agree to hold Dravet Syndrome Foundation and its staff, officers, and volunteers entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence. I swear that my child is participating voluntarily, and that all risks have been made clear to me. Additionally, my child does not have any conditions that will increase their likelihood of experiencing injuries while engaging in this camp. By signing below I forfeit all right to bring a suit against the Dravet Syndrome Foundation for any reason. In return, my child may participate in Super Siblings Camp. I will also make every effort to make sure that my child obeys safety precautions. I will ask for clarification if needed. In an emergency, I acknowledge that I am solely responsible for all medical and other costs arising out of bodily injury or any loss sustained through participation in this activity. I authorize program staff to secure any licensed hospital, physician and/or medical personnel any treatment deemed necessary for the participant’s immediate care. I have read the above carefully before signing.
